Deadline to complete the Get Healthy Know Your Numbers Wellness Screening is December 31, 2017.
This is a friendly reminder to anyone who has not yet registered for or completed the wellness screening. Employees only must complete a wellness screening to earn the Get Healthy contribution waiver beginning July 1, 2018. This year the PEBTF is not requiring spouses to have the screening done.
The Wellness Screening:
Our on-site event will take place on November 7, 2017 at the Convocation Center. If you plan to attend our event you must register by October 24, 2017.
If you choose to visit your physician during work hours, you must use appropriate leave and your physician will need to complete the Physician Results Form. The form must be faxed to Quest Diagnostics by December 31, 2017 in order for you to qualify as completing the wellness screening. You have to go to the PEBTF’s website to obtain the form. It includes your pre-printed information and a barcode. This is the only form that will be accepted.
To register for a screening or to obtain the Physician Results Form:
· Go to www.pebtf.org
· Click on the Get Healthy logo
· Scroll to the bottom of the next page and click on “Continue to Quest Diagnostics Health & Wellness"
· Complete a new registration (even if you registered last year)
· The Registration Key is PEBTF
· Make your selection (go to Quest, attend an on-site event, obtain the Physician Results Form)
