What is Involved?
· Before your health center visit, you will be asked to complete a confidential survey that takes 10 – 20 minutes. After your visit, you will be asked to complete a short satisfaction survey that takes 5 minutes. You will be given a $15 bank card. (The card can be taken to PNC to get the $15 or it can be used at any location that accepts cards.) You will be asked if you are interested in participating in two follow-up surveys.
· You will be contacted again in 4 months for a follow-up survey that will take
10 – 20 minutes. Once completed, $25 will be added to your bank card.
· You will be contacted again in 12 months for a follow-up survey that will take between 10 – 20 minutes. Once completed, $40 will be added to your bank card.
Study will begin on Thursday, January 28, 2016.
