SPACE UTILIZATION REQUEST

The Space Utilization Request Form is to be completed when any employee (new or relocated), material, or equipment is to be placed in a room outside of the original location.  This form is to be completed in its entirety, including signatures, and submitted to Facilities Management (FM) at least three (3) months prior to expected move/relocation (academic departments must also copy the Space Utilization Requests to Dr. Stephen Whitehead, Associate Provost).  Once received by FM, it will be reviewed to determine the feasibility and appropriate location of the request.  Failure to properly complete the form in its entirety will result in the request being returned to the requestor for proper completion.

 

Upon FM’s approval of the Space Utilization Request form, it will be forwarded to both the VP of Administration and Finance and the Provost for their input/denial/approval.  Once FM receives approval, the request will be placed in the queue and direction will be given to the requestor.
