TO:  All SCUPA and AFSCME Employees

 

This is a friendly reminder to anyone who has not yet registered for the wellness screening. Employees must complete a wellness screening as one of the requirements to earning the Get Healthy contribution waiver beginning July 1, 2014.

        

 The Wellness Screenings, which will be offered from September 16, 2013 through January 31, 2014:

 

·         Include a blood test for cholesterol and blood glucose (sugar) levels and height, weight and blood pressure measurements

·         Are free at an onsite event or a Quest Patient Service Center (you may also obtain the screening at your doctor’s office but you will have to pay a copay)

·         Are fast – takes about 15 minutes

·         Are easy to schedule – online at www.pebtf.org (Click on the Get Healthy logo) or by phone at 866-908-9440.

 

If you choose to visit your physician, you must use appropriate leave and your physician will need to complete the Physician Result Form.  The form must be faxed to Quest Diagnostics by January 10, 2014 in order to qualify as completing the wellness screening.

 

In order to earn the lower contribution rate:

ü  Employees must complete a wellness screening AND

ü  The employee and, if applicable, spouse/domestic partner must complete the annual Get Healthy Assessment, which will be offered in April, 2014.
