Important Information About the PEBTF’s Get Healthy Program and the Contribution Waiver Requirement for July 1, 2014
Completion of the Health Assessment NOT Required
Employees will earn the health care contribution waiver for the period July 1, 2014 to June 30, 2015 if they successfully completed a Know Your Numbers wellness screening before January 31, 2014 (for a Physician Results Form, testing had to be completed between January 1, 2013 and January 1, 2014 and faxed to Quest Diagnostics by January 10, 2014).
Completion of a Health Assessment by the employee and covered spouse/domestic partner in spring 2014 is NOT required.
The Get Healthy Program requirements are subject to change in the future and this change is effective for this waiver period only. Any changes will be communicated to you by the PEBTF via mail and email in advance of the change.
