December 31, 2014 is the last day for the Get Healthy Wellness Screening.
If you have not already done so, you must register for and obtain the wellness screening in order to receive the lower contribution rate for your health coverage. The current participant rate is 2% and non-participant rate is 5% of your gross salary. These rates could increase July 1, 2014.
To register for the screening, either with your physician or at a Quest Diagnostics site, you’ll need to follow these steps:
ü Go to www.pebtf.org
ü Click on the Get Healthy logo on the right-hand side
ü Scroll to the bottom and click on “Continue to Quest Diagnostics Blueprint for Wellness”
ü Under New Participant enter the Registration Key: PEBTFPSC
ü On the next screen enter your information and click on “Submit”
ü You will then be asked to select whether you will go to your doctor or to a Quest Diagnostics site (you can schedule an appointment or choose walk-in)
If you prefer to have your doctor perform the screening, your doctor will need to complete the Physician Results Form and fax it to Quest Diagnostics by December 31, 2014. You can get the form by contacting the Human Resources Office.
