MetLife recently sent out packets to employees asking to designate a beneficiary or beneficiaries.

 

If you received the notice, you can submit the information in one of two ways:    

           

Ø  Complete the Group Term Life Insurance Beneficiary Designation form and send to:

§  MetLife Record Keeping Center

§  PO Box 14401

§  Lexington  KY   40512-4401

 

Ø  Complete your designation(s) on-line by going to www.metlife.com.  You must register by creating a user name and password.  Please note, our employer is listed as Commonwealth of Pennsylvania.

 

 

(The reason for their mailing:  In January of 2013, the group life carrier switched from Prudential to MetLife.  We were informed that beneficiary information was transferred to the new insurance company.  Apparently, that did not take place creating the need for MetLife to reach out to anyone who had not submitted beneficiary information after the transition.)
