Registration for the Get Healthy screening is now open. Please follow the directions below to register online or by phone.
To register online:
ü Go to www.pebtf.org
ü Click on the Get Healthy logo on the right-hand side of the home page.
ü Click on “Continue to Quest Diagnostics Blueprint for Wellness”.
ü Register as a new user, even if you have registered in previous years. Click on the “Register Now” link in the “Not Registered Yet” yellow box. Enter registration key PEBTF (which is not case-sensitive).
ü Enter your Social Security number and date of birth. Create an account by entering a username and password. (Please note: an e-mail address is required for account set up. If you do not have an e-mail address you may register by phone.)
ü Schedule your wellness screening at an onsite event (The event will take place on campus on Thursday, December 17th, 2015 in the South Wing of the Convocation Center) or a Quest Diagnostics Patient Service Center. (You can view locations only on this website.)
ü Print the confirmation page if you prefer. You will receive a confirmation e-mail. You will also receive a reminder e-mail 24 hours before your scheduled wellness screening.
ü After your screening, you will receive an e-mail notice when results are available to review online. If you obtain the screening at a Patient Service Center or if you submit a Physician Results Form, you will receive a printed report in approximately two to three weeks after your screening or after you submit the Physician Results Form. This report is verification that your results were received by Quest Diagnostics.

Call Quest Diagnostics at 1-855-623-9355 (Monday – Friday, 8:00 a.m. – 9:30 p.m. and Saturday 8:30 a.m. – 5:00 p.m. EST).
