The PEBTF’s Know Your Numbers Wellness Screening Program begins August 1, 2019.   (Registration began on July 1st)

 

Deadline to complete the Get Healthy Know Your Numbers Wellness Screening is December 31, 2019.

 

There are 3 ways to obtain your screening:

 

·       Onsite screening at select Commonwealth worksites

·       Quest Diagnostics® Patient Service Center

·       Your doctor’s office.

 

To register for a screening:

 

Ø  Go to www.pebtf.org

Ø  Click on the Get Healthy logo on the left side, scroll down and click on “Continue to Quest Diagnostics Health & Wellness”, at the next screen click “Ok”, log in or create an account, then follow the directions accordingly.

 

Onsite Event at Cal U:

 

Our onsite event will be held on Wednesday, October 9, 2019 from 8:30 a.m. to 12:00 p.m. in the South Wing of the Convocation Center.  Signs will be posted directing attendees to the appropriate rooms.  You can register for this event until September 25, 2019 at midnight.

 

The Wellness Screening:

 

ü  Includes a blood test for cholesterol and blood glucose (sugar) levels and height, weight and blood pressure measurements

ü  Is free at an onsite event or a Quest Patient Service Center (you may also obtain the screening at your doctor’s office but you will have to pay a copay)

ü  Is fast – takes about 15 minutes

ü  Is easy to schedule – online at www.pebtf.org (Click on the Get Healthy logo and continue to the Quest Diagnostics Site) or by phone at 855-623-9355 to register Monday – Friday, 8:00 a.m. to 9:30 p.m. and Saturday 8:30 a.m. to 5:00 p.m.

ü  Spouses/Domestic Partners are not required to participate this year

 

If you choose to visit your physician during work hours, you must use appropriate leave and your physician will need to complete the Physician Results Form.  The form must be faxed to Quest Diagnostics by December 31, 2019 in order for you to qualify as completing the wellness screening.  You must obtain the Physician Results Form through the PEBTF Get Healthy site.  The form will be populated with your information (not your social security number) and will include a barcode allowing for accuracy.
