The Pennsylvania Employees Benefit Trust Fund offers, free of charge, confidential
wellness screenings that give you valuable information about your health and health
risks. After the screening you may visit your doctor to discuss your results and
set goals for better health. Your health and well-being are important to the PEBTF.
If each of us manages our health issues and maintains a healthy lifestyle, we can
positively impact our total health care costs. The wellness screening is a requirement
towards earning the health care contribution waiver.
Newly Enrolled or Return to Benefits: If you are a newly enrolled employee or an employee re-enrolled for benefits after
a break in coverage, you will receive a letter from the PEBTF giving you 45 days from the date of the letter
to complete a Wellness Screening. Completion of this Wellness Screening will lower
your contribution rate.
To register by phone:
- Call Quest Diagnostics at 1-855-623-9355 (Monday - Friday, 8:00 a.m. - 9:30 p.m.,
Saturday 8:30 a.m. - 5:00 p.m. EST).
To register on-line:
- Go to www.pebtf.org
- Click on the Get Healthy logo on the right-hand side of the home page.
- Click on "Continue to Quest Diagnostics Blueprint for Wellness".
- Register as a new user by clicking on the "Register Now" link in the "Not Registered
Yet" link in the yellow box. Enter registration key PEBTF(which is not case-sensitive).
- Enter your Social Security Number and Date of Birth. Create an account by entering
a username and password. (Please note: an e-mail address is required for account set-up. If you do not have
an e-mail address you may register by phone.)

- Schedule your wellness screening at an onsite event (On-campus events usually take
place in the fall, a campus-wide announcement is sent out with details) or a Quest
Diagnostics Patient Service Center. You can view locations on their website (www.questdiagnostics.com).
- You will receive a confirmation e-mail, which you may want to save or print. You
will also receive a reminder e-mail 24 hours before your scheduled wellness screening.
- After your screening, you will receive an e-mail notice when your results are available
to review on-line. If you obtain the screening at a Quest Diagnostics Patient Service
Center or if you submit a Physician Results Form, you will receive a printed report
in approximately two to three weeks after your screening or after you submit the Physician
Results Form. This report is verification that your results were received by Quest
Diagnostics.
